Experts feel that treating erectile dysfunction on your own, without consulting a doctor, is unsafe. "If you have ED, the first thing you need is a diagnosis," says impotence expert Steven Lamm, MD, a New York City internist and the author of The Hardness Factor (Harper Collins) and other books on male sexual health. He says men with severe erectile dysfunction probably need one of the prescription ED drugs, which include the PDE-5 inhibitors Levitra (vardenafil), Cialis (tadalafil), and Stendra (avanafil), as well as Viagra. But, he says, mild ED -- including the feeling that "you're not as hard as you could be" -- often responds to natural remedies.
On the horizon is gene therapy that would deliver genes that produce products or proteins that may not be functioning properly in the penile tissue of men with ED. Replacement of these proteins may result in improvement in erectile function. Experimental animal models have demonstrated improvement in erectile function with gene therapy. Human studies may also demonstrate success with this therapy. Gene therapy may take a long time for regulatory approval and public acceptance.

A German study found that 69% of men involved in a sleep apnea study also suffered from ED. Disruptions to breathing during sleep affect oxygen levels in the blood, which can cause problems generating an erection. Lack of sleep also disrupts hormone production.
The most common side effects of Sildenafil: headache; flushing; upset stomach; abnormal vision, such as changes in color vision (such as having a blue color tinge) and blurred vision; stuffy or runny nose; back pain; muscle pain; nausea; dizziness; rash.
There is some evidence that acupuncture can be helpful in treating erectile dysfunction. Although studies show mixed results, one study found that after 4–10 acupuncture sessions, 13 of the 19 patients in the intervention group were able to obtain a full erection without further therapy and their symptom scores improved by 41.9 percent. This study suggests that acupuncture can be helpful for some men in improving erectile function. (31)
Another second-line therapy is the use of alprostadil, either intracavernosal or intraurethral. Two products are available for direct injection, Caverject (Pfizer) or Edex (Actient). A small needle is used to inject the medication into the lateral aspect of the penis through a small-gauge needle.

Treatments are available to help you with both physical and psychological causes . Sexual dysfunction is normally treated by correcting or addressing the underlying problem or problems that are causing the dysfunction. If the cause is physical, the treatment will likely be medication or mechanical aids. If it is psychological, counseling, therapy, behavior modification and education may work. Some people with sexual dysfunction may have both physical and psychological causes, and in that case, both will need to be treated. If you have a medical condition that is causing your dysfunction, your doctor will work with you to correct that problem. A common cause is hormonal imbalance. Both men and women can have hormone therapy to help them restore proper sexual function. There are also drugs, like Viagra (generic name sildenafil), that can help increase blood flow to the penis. If your dysfunction is related to a medication you're taking, your doctor may be able to prescribe a different medication that won't interfere with your sexual activity. Men who have difficulty getting or maintaining an erection may have success with special vacuum devices, or penis pumps. Penile implants are an option for some men, especially those who have lost sensation due to spinal injury. Women who have vaginismus can sometimes be treated with special dilators that can help relax the muscles in the vagina. Both psychotherapy and sex therapy can help with the psychological causes of sexual dysfunction. Counselors can help people work through fear, stress, anxiety, past trauma, body image issues, and other issues that can keep people from a fulfilling sex life. Sex therapists can help couples who are experiencing sexual problems as well. Sex therapists can help couples delve into problems in a way that most physicians aren't equipped to do. Both counselors and sex therapists may recommend education, practicing communication, and behavioral treatments that can address underlying issues and help individuals and couples find techniques that work for them.
