In the 14 years or so since Viagra was introduced in 1998, the number of men diagnosed with erectile dysfunction (ED) has increased by a whopping 250 percent. Men are realizing more and more that they’re not alone and that they have options. Aside from pills like Viagra, Levitra, and Cialis, for example, there are surgical, therapeutic, even do-it-yourself treatments that can bring back that lovin’ feeling. "Some older ED treatments are actually being used more than ever, especially for men who can't take the pill," says Michael Feloney, MD, a urologist at the Nebraska Medical Center in Omaha. Psychosexual counselling, or sex therapy, is an appropriate recommendation especially for men who are experiencing discord with their partner especially if the conflict is related to the man’s ED. Counselling usually consists of 5–20 sessions with counsellor. It is our recommendation that referral doctors treating men with ED make a referral to a psychotherapist or sex therapist who is certified by AASECT (American Association of Sexuality Educators, Counselors and Therapists) of certified sexuality educator.43
Often known by the common brand name Cialis, tadalafil is a PDE-5 inhibitor that you take once per day to provide 18-36 hours of effectiveness. This significantly reduces the planning typically involved in using other treatments like sildenafil. The results of a study looking at men using sildenafil over the course of four years suggest that it’s safe and effective when used over a prolonged period of time. Coronary Artery DiseaseCoronary artery disease (CAD) and ED frequently occur concomitantly due to mutual risk factors such as diabetes, hypertension, and hyperlipidemia. In men with CAD, the incidence of ED is estimated to be 42% to 57%. Because the primary cause of ED is vasculogenic, ED can precede CAD, stroke, and peripheral artery disease by an average of 3 years.27,28 The package insert for PDE5Is cautions providers about using these agents in patients with uncontrolled hypertension (>170/100 mmHg), CAD with unstable angina, or a history of myocardial infarction (MI), stroke, or life-threatening arrhythmia within the past 6 months.8-11 The primary concern with using PDE5Is in these patients is that the physical demands of intercourse could provoke a CV event.
